Have you ever seen something unsafe – and not done anything about it? All unsafe acts or conditions should be reported to your supervisor immediately, even if no one was hurt! If you don’t report it, someone could get hurt – and that someone maybe you or your co-worker. 

In this webinar, you will learn the complete understanding of how to properly conduct an accident/incident investigation to drive to the root cause. Items to be covered include:
 
When investigating accidents – the more details the better. You should also investigate as soon as you can, so conditions don’t change. A complete accident investigation determines the following: 

  • What happened? 
  • When did it happen? 
  • Where did it happen? 
  • Who was involved? 
  • Why did it happen? 
  • How can it be prevented from happening again? 
  • The observations of co-workers can be critical and if you are interviewed, heed the following:
    • Make mental or written notes about the accident before the investigation starts. 
    • Avoid talking to others before talking to the investigator, since this may confuse the facts. 
    • Answer all questions about the incident as accurately as possible. 
    • Don’t change anything at the scene of the accident.
